Added the ability of attaching the uploaders username to the record data uploaded

currently the user can attached optionally which is decided by a checkbox with a disclaimer.

If we don't want to provide the users the option of abstaining from attaching their username to the data, the checkbox can be removed from the pr.

checkbox div and disclaimer styled with gemini fast model (claude was down :( )
