[BUG] VS Code Hangs when elicitation is requested with empty schema (i.e. user info not requested)

Previously, VS code used to show a yes/no dialog when no info was requested by the elicitation schema in the MCP server. Now in the latest version it just hangs indefinitely on the MCP tool operation.

This is a regression; a proper error message should flow here at the least from VS code side. 
It currently regresses the existing implementations which are supported by the MCP spec.
